What CIPROBAY 250 and what it is used?

Ciprofloxacin, the active ingredient in CIPROBAY 250, belongs to the group of quinolones. The main site of action of quinolones is on the bacterial enzyme (gyrase), which plays a vital role in bacterial metabolism and reproduction. By blocking this enzyme with ciprofloxacin (gyrase inhibitor) medication has a bactericidal effect on the causes of disease (ie kills microorganisms).

Indications:

For the treatment of uncomplicated and complicated infections caused by pathogens susceptible to ciprofloxacin:

Infections:

The respiratory system. Most microorganisms known as "problematic organisms" (eg Klebsiella, Enterobacter, Proteus, Pseudomonas, Legionella, Staphylococcus, Escherichia coli) are very sensitive to CIPROBAY. Most cases of pneumonia who do not require hospitalization are caused by Streptococcus pneumoniae. In these cases CIPROBAY 250 is not a drug of first choice.
The middle ear (otitis media) and paranasal sinuses (sinusitis), especially when caused by problematic pathogens such as Pseudomonas or Staphylococus. Another antibiotic should be used for the treatment of acute tonsillitis (angina).
 Eye
Renal and / or efferent urinary tract
The sexual organs, including inflammation of the ovaries and fallopian tubes (adnexitis), gonorrhea, and infections of the prostate (prostatitis)
CIPROBAY 250 is not effective against Treponema pallidum "(the causative agent of syphilis)
The abdomen, eg. gastrointestinal tract, biliary tract and peritoneum (peritonitis)
 Skin and soft tissue
Bones and joints. Blood poisoning (sepsis)

Infection or risk of infection (prophylaxis) in patients with compromised immune systems, eg. who are treated with drugs that suppress the natural immune defenses (immunosuppressants) or whose blood contains a reduced number of certain white blood cells (neutropenia). Pinpoint removal of bacteria from the gut (selective intestinal decontamination) during treatment with drugs suppress the immune system (immunosuppressants). landfills and for adolescents between the ages of 5 and 17 years: Treatment of acute oral infections in cystic fibrosis (cystic fibrosis, congenital metabolic disease with increased production and increased viscosity of mucus in the bronchial glands and digestive system ) caused by P. aeruginosa, treatment is sufficient or effective parenteral treatment, which is difficult to implement. CIPROBAY 250 is not recommended for other indications.

Anthrax:

For immediate treatment for the treatment of anthrax after inhaling anthrax bacillus {Bacillus anthracis). The efficacy of Ciprobay for anthrax has been demonstrated in studies.

What you should know before using CIPROBAY 250?

CIPROBAY 250 should not be used:

If you are hypersensitive (allergic) to ciprofloxacin and other drugs from the same pharmacological group (quinoline group gyrase inhibitors) or other ingredients CIPROBAY 250.
If you are pregnant or breastfeeding.

Particular attention is necessary when used CIPROBAY 250: If you suffer from seizures (epilepsy) or other previous damage to the central nervous system (eg increased susceptibility to seizures of seizures, decreased cerebral blood flow, brain injury or stroke past). Patients in this category are at risk of side effects on the central nervous system. In isolated cases appear psychotic reactions (mental perception affected by damage to the level of a suicide attempt), in some cases after the first use. In these cases, immediately stop using and inform CIPROBAY lekar.ako treating a severe and persistent diarrhea during or after treatment. You should consult a doctor as this could be a sign of serious, possibly life-threatening intestinal condition (pseudomembranous colitis) that requires immediate treatment. Using CIPROBAY should be discontinued in these cases and appropriate treatment (eg oral vancomycin 250 mg 4 x daily). Do not take drugs that suppress gastric motility. In isolated cases were observed inflammation of the tendons (tendinitis) and tendon rupture (eg Achilles tendon) after treatment with fluoroquinolones (pharmacological group belongs CIPROBAY 250). These findings were observed mainly in elderly patients who have previously treated with corticosteroids. If it is suspected tendinitis, 250 CIPROBAY treatment should be stopped immediately to avoid physical exertion and to implement appropriate lechenie.-Although photosensitivity occurs very rarely following treatment with ciprofloxacin-treated patients 250th CIPROBAY not be exposed to sunlight without the need for downtime and avoid ultraviolet light (high mountain sun, tanning beds). Treatment should be stopped if there are photosensitive reactions (eg skin reactions similar to sunburn). Isolated cases a severe allergic quick reactions, including facial swelling (edema), blood vessels and larynx and breathing difficulties (dyspnea), extending to life-threatening shock (anaphylaxis / anaphylactoid) reactions, in some cases after the first use. In such cases, stop using CIPROBAY 250 immediately and tell your doctor.

Children and adolescents

As with other gyrase inhibitors, known to ciprofloxacin, the active ingredient in CIPROBAY 250, can cause damage to weight-bearing joints of juvenile animals. Assessment of safety data in patients under 18 years, who suffered from cystic fibrosis mainly (CF) does not reveal evidence of impaired joints / cartilage. Current data support the use of CIPROBAY to treat episodes of acute infections in cystic fibrosis caused by P. aeruginosa in children and adolescents between 5 and 17 years, at present there is insufficient experience with its use in children and adolescents with other infections, and children under 5 years. Therefore, ciprofloxacin should not be used for other infections in children under 5 years.

Pregnancy

CIPROBAY 250 should not be used during pregnancy as there is no data on its safety in pregnant women. Animal studies do not provide evidence of fetal malformations (teratogenic effects), but not completely ruled out that the cartilage damage is caused to organisms that have not reached maturity.

Breastfeeding

It is generally recommended CIPROBAY 250 should not be used while breastfeeding.

Driving and operating machinery

Do not drive or operate machinery while taking this medication, even when used properly the medicine could harm the speed of response and thus its ability to drive, use machines or work without safe foothold, or patients can not perform these activities at all. This is particularly true for the beginning of treatment when the dose is increased, a change in medication in conjunction with alcohol.

Interactions with other medicines:

Ciprobay / products containing iron / antacids and products with high buffering capacity, containing magnesium, aluminum or calcium. Concomitant use of CIPROBAY 250 and the above-mentioned products reduce the absorption of ciprofloxacin, the same applies as for sucralfate antiviral drug substance containing didanosine oral solution food, drinks fortified with minerals and large amounts of dairy products. Therefore Ciprobay 250 should be administered either 1-2 hours before or at least 4 hours after these products. This restriction does not apply to antacids such as H2 blockers.

Ciprobay / xanthine

Concomitant administration of CIPROBAY 250 and theophylline (asthma) may lead to unwanted increase of the concentration of theophylline in the blood and therefore an increased incidence of adverse effects caused by theophylline, which in isolated cases can be life threatening or fatal. If necessary the use of both drugs at the same time, the concentration of theophylline blood levels should be monitored and the dosage should be reduced. There are reports of elevated concentrations of xanthine derivatives caffeine and pentoxifylline (a medicine that improves blood circulation) in the blood, when these substances are administered concomitantly with Ciprobay 250.

Ciprobay / non-steroidal anti-inflammatory drugs

Animal studies have shown that using a combination of very high doses of quinolones (gyrase inhibitors) and certain drugs that inhibit inflammation (non-steroidal anti-inflammatory drugs) can cause seizures. This does not apply to medicines containing aspirin.

Ciprobay / cyclosporine

Temporary impairment of renal function associated with an increase in the concentration of creatinine in the blood was observed in isolated cases where CIPROBAY 250 was administered at the same time with ciclosporin (a medicine that suppresses the body's defense mechanisms). If you take both drugs at the same time should be carefully monitored (twice a week) your creatinine concentration.

Warfarin

Concomitant use of CIPROBAY 250 and warfarin (a medicine that reduces blood clotting) may increase the effect of warfarin.

Ciprobay / glibenclamide

In isolated cases, the simultaneous use of CIPROBAY 250 and glibenclamide (diabetes) may enhance the effects of glibenclamide to the extent of hypoglycaemia

Ciprobay / probenecid

Probenecid (gout) affect the excretion of ciprofloxacin in urine (renal secretion). Concomitant use of probenecid CIPROBAY 250 and increases the concentration of ciprofloxacin in the blood (serum).

Ciprobay / metoclopramide

Metoclopramide (stomach medicine) accelerates the absorption of ciprofloxacin in the blood and increases the maximum concentration in the blood (plasma) faster than usual. It is unknown effect on the bioavailability of Ciprobay the human body.

Ciprobay / mexiletine

Concomitant use of these drugs may lead to increased concentrations of mexiletine in the body.

SgrgoYau / fenitosh

Increased or reduced serum concentrations of phenytoin have been reported following concomitant use of these two drugs.

Ciprobay / diazepam

It has been reported that concomitant use of CIPROBAY and diazepam diazepam slows down the body (reduced clearance, a prolonged half-life). Therefore, careful monitoring of treatment with diazepam.

Ciprobay / methotrexate

Concurrent use of these substances can lead to delayed excretion of methotrexate and thus to increased plasma levels of methotrexate. These patients should be carefully monitored, as this may lead to an increased risk of toxic reactions induced by methotrexate.

Ciprobay / omeprazole

Concomitant use of tsiorofloksatsin omeptazol and can lead to a slight increase in peak plasma concentrations (Cmax) and bioavailability (AUK) of ciprofloxacin. Please tell your doctor or pharmacist if you are taking other medicines or other medicines you are taking at present, even if they are medicines obtained without a prescription.

 How to use CIPROBAY 250?

CIPROBAY 250 should always be taken in accordance with the instructions of your doctor. Ask your doctor or pharmacist if you are not sure how to take this medicine.

In patients with particularly severe, life-threatening infections (particularly those caused by Pseudomonas, Staphylococcus and Streptococcus), for example. pneumonia caused by Streptococcus, recurrence of infection in patients with cystic fibrosis (cystic fibrosis, congenital metabolic disease with increased production and increased viscosity of mucus in the bronchial glands and digestive system), infections of bones and joints, blood poisoning (sepsis) and infections the peritoneum (peritonitis), usually given intravenously Ciprobay. Alternatively, the patient can be treated by giving a dose of 2 x 750 mg (equivalent to 2x3 tablets Ciprobay 250). Patients with urinary tract or genitals caused by Chlamydia, if necessary, the daily dose may be increased to 2 x 750 mg (equivalent to 2x3 tablets Ciprobay 250).

Anthrax:

Adults: 2 tablets Ciprobay 250 (ciprofloxacin 500 mg) two times a day

Children: 15 mg / kg bw 2 times a day. Maksimalnatadnevna dose should not exceed 500 mg

Note:

Besides Ciprobay 250 available and other forms of oral administration containing lower or higher doses of drug and infusion solutions for intravenous therapy. If the patient is unable to swallow tablets, it is recommended to start treatment with ciprofloxacin for intravenous infusion therapy.

Elderly

Elderly patients should receive as much as possible at a lower dose, consistent with the severity of infection and their renal function (creatinine clearance).

Children and adolescents

The recommended oral dose for acute infections caused by P. aeruginosa in patients with cystic fibrosis (an inherited metabolic disease with increased production and increased viscosity of mucus in the bronchial glands and digestive system) has daily 2 x 15 (-20) mg / kg ( maximum 1500 mg / day).

Patients with impaired renal or hepatic function:

Adults

1. The following doses are recommended for moderate to severely impaired renal function - In patients with creatinine clearance between 31 ml / min and 60 ml / min (serum creatinine between 1,4 mg/100 ml and 1,9 mg/100 ml), maximum dose for oral administration is 1000 mg ciprofloxacin twice daily.

2. Patients with impaired renal function who are undergoing hemodialysis should receive the same dose after each dialysis, and patients with moderate to severe renal impairment.

3. In patients with impaired renal function who are in continuous ambulatory peritoneal dialysis (CAPD), ciprofloxacin 500 mg (equivalent to - 2 tablets Ciprobay 250) is required 4 x daily 6-hour intervals for peritonitis. Alternatively Ciprobay infusion solutions can be added to (intraperitoneal) dialysate. The dosage is 50 mg of ciprofloxacin per liter dialysate 4 x 6-day time intervali.Ima limited clinical experience with a small number of patients with this indication. We use high doses Ciprobay to maintain sufficiently high concentrations of ciprofloxacin in the peritoneum. Patients should be closely monitored for side effects in this regard. If a clinically important side effect or symptom of overdosage, the dosage should be reduced or use of Ciprobay be discontinued.

No need to adjust the dose in patients with impaired hepatic function.

5. In patients with impaired renal and hepatic function, the dose should be adjusted with impaired renal function, it may be necessary to monitor the concentration of ciprofloxacin in the blood.

Children and adolescents

No information on the effect of renal and hepatic function on dosing in children and adolescents.

When and how to take Ciprobay 250?

The tablets should be swallowed whole with liquid. Tablets should not be taken with meals. Take tablets on an empty stomach accelerates the absorption of the drug. Tablets should not be taken together with a dairy or beverage enriched in minerals (eg, milk, yogurt or orange juice fortified with calcium). Drug absorption is not affected by foods containing calcium.

How long should I use Ciprobay 250?

Your doctor will decide how long you will take Ciprobay 250. This will depend on the severity of your infection and how you respond to treatment on your general condition and sensitivity of the organism causing the infection to the active ingredient ciprofloxacin. Treatment should always be continued for at least 3 days after fever reduction and resolution of clinical signs.

As a rule, the average duration of treatment:

Adults

1 day for acute uncomplicated gonorrhea and cystitis - up to 7 days for infections of the kidneys, urinary tract and abdominal cavity - in patients with compromised immune systems, treatment should be continued, while reducing the total number of white blood cells - the maximum 2 months for inflammation of bone (osteomyelitis) - 7-14 days in all other infections. In streptococcal infections, treatment should continue for at least 10 days because of the risk of complications. Chlamydia infections should also be treated for at least 10 days.

Children and adults aged 5 to 17 years

10-14 days for episodes of acute infections of cystic fibrosis caused by P. aeruginosa.

Anthrax

60 days for immediate treatment for the treatment of infections after inhaling anthrax batsili.Molya consult your doctor or pharmacist if you consider the effects of Ciprobay 250 are very strong or very weak.

If you take more than prescribed Ciprobay 250:

Several cases of transient (reversible) renal impairment have been reported after administration of very high doses. In these cases, therefore, renal function should be checked by a doctor. Application of products containing magnesium or kaltsiyneutralizirat stomach acid and thus reduce the absorption of ciprofloxacin in the bloodstream.
