ESTSITIL WHAT IS AND WHAT IT IS USED
Estsitil contains escitalopram (escitalopram) is used for treating depression (major depressive disorder) and anxiety disorders (such as panic disorder with or without agoraphobia, ie fear of places where you can not help, social anxiety disorder, generalized anxiety disorder and obsessive compulsive disorder).
 
Escitalopram belongs to a group of antidepressants called selective serotonin reuptake of serotonin (SSRIs). These drugs act on the serotonin system in the brain by increasing serotonin levels. Disturbances of serotonin system is considered an important factor in the development of depression and related disorders.
 

 BEFORE YOU TAKE ESTSITIL

 
Do not take Estsitil
 
 if you are allergic (hypersensitive) to estsitaloprom or any of the other ingredients of Estsitil (see section 6 "Further information").
 if you are taking other medicines that belong to a group called MAO inhibitors, including selegiline (used to treat Parkinson's disease), moclobemide (used to treat depression) and linezolid (an antibiotic).
 
Take special care with Estsitil
 
Please tell your doctor if you suffer from any other condition or disease as may be necessary for your doctor to take this into account.
 
In particular, tell your doctor:
 
 if you suffer from seizures for the first time in your life or have epilepsy. Estsitil therapy should be discontinued if seizures occur or if there is an increase in seizure frequency (see also section 4 "Possible side effects").
 if you have liver or kidney function. You need to your doctor to adjust your dose.
 if you have diabetes. Estsitil treatment may alter glycemic control. It may be necessary to adjust the dose of insulin and / or oral hypoglycaemic drugs.
 have a reduced level of sodium in the blood.
 If you have a tendency to bleed easily or bruising.
 If you are receiving electroconvulsive therapy.
 if you have coronary heart disease.
 
Please note
 
As with other drugs used to treat depression and related disorders, the improvement is not achieved immediately. After initiation of therapy with Estsitil may take several weeks before you feel better. In the treatment of panic disorder, improvement usually occurs after about 2-4 weeks. At the beginning of treatment, some patients may experience increased anxiety, which will disappear with longer treatment. It is very important to carefully follow your doctor's instructions and do not stop treatment or change the dose without consulting him.
 
Thoughts of suicide and worsening of your depression or anxiety disorder.
 
If you are depressed and / or have anxiety disorders you can sometimes have thoughts of harming or killing themselves. These may be increased when first starting antidepressants, so as with all these drugs take time to start running - usually about two weeks but sometimes longer.
 
You may be more likely to think like this:
 
 If you have previously had thoughts about killing or harming yourself.
 If you are a young person. Clinical studies have shown an increased risk of behavior associated with suicide in young people (under 25 years) with psychiatric conditions who were treated with an antidepressant.
 
If at any time until you get any thoughts of harming or killing yourself, contact your doctor or go to hospital.
 
You may find it helpful to tell a relative or close friend that you are depressed or have an anxiety disorder, and ask them to read this leaflet. You can ask them to tell you if they think your depression or anxiety is getting worse, or are worried about changes in your behavior.
 
Some patients with manic depressive illness may enter into a manic phase. It is characterized by unusual and rapidly changing ideas, inappropriate happiness and excessive physical activity. If you experience any of these, contact your doctor. Symptoms such as restlessness or difficulty sitting or standing in one place can also occur during the first weeks of treatment. Tell your doctor immediately if you experience these symptoms.
 
Sometimes you may not be aware of the above symptoms and therefore may be to your benefit to ask a friend or relative to help you keep track of possible signs of change in your behavior.
 
Tell your doctor immediately or contact your nearest hospital if you have disturbing thoughts or experiences, or if any of the above symptoms occur during therapy.
 
Use in children and adolescents under 18 years of age
 
Usually Estsitil should not be used in children or adolescents under 18 years of age.
 
We also know that patients under 18 have an increased risk of side effects such as suicide attempt, suicidal thoughts and hostility (mostly aggression, oppositional behavior and anger) when they take this class of drugs. However, your doctor may prescribe Estsitil for patients under the age of 18, when he decided that it is in their interest. If your doctor has prescribed Estsitil a patient under 18 and you want to discuss this, please go back to your doctor. You must tell your doctor if any of the above symptoms occur or worsen when patients under 18 Estsitil accept. Also current term safety consequences on growth, maturation and cognitive and behavioral development of Estsitil in this age group have not yet been demonstrated.
 
Taking other medicines
 
Please tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including medicines obtained without a prescription.
 
Tell your doctor if you are taking any of these medicines:
 
 "Non-selective monoamine oxidase inhibitors (MAOIs)" containing phenelzine, iproniazid, isocarboxazid, nialamide and tranylcypromine as active substances. If you are taking any of these medicines, you have to wait 14 days before you start taking Estsitil. Estsitil after stopping must pass seven days before taking any of these medicines.
 
 "Reversible, selective MAO-A inhibitors", containing moclobemide (used to treat depression).
 "Irreversible MAO-B inhibitors", containing selegiline (used to treat Parkinson's disease). They increase the risk of side effects.
 antibiotic linezolid.
 Lithium (used to treat manic depressive disorder) and tryptophan.
 Imipramine and desipramine (both used to treat depression).
 Sumatriptan and similar medicines (used to treat migraine) and tramadol (used for severe pain). They increase the risk of side effects.
 Cimetidine and omeprazole (used to treat stomach ulcers), fluvoxamine (an antidepressant) and ticlopidine (used to reduce the risk of stroke). They can cause elevated blood levels of Estsitil.
 St. John's wort (hypericum perforatum) - a herbal medicinal product used for depression.
 Aspirin and non-steroidal anti-inflammatory drugs (medicines used to relieve pain or to thin the blood, so called anticoagulants). They can increase the tendency to bleed.
 Warfarin, dipyridamole, and phenprocoumon (medicines used to thin the blood, so called anticoagulants). Your doctor will probably check the clotting time of your blood at the beginning and end of Estsitil to check whether your dose anticoagulation is still sufficient.
 
 Mefloquine (used to treat malaria), bupropion (used to treat depression) and tramadol (used to treat severe pain) because of the possible risk of lowering the seizure threshold.
 Neuroleptics (medicines to treat schizophrenia, psychosis) because of the possible risk of lowering the seizure threshold, and antidepressants.
 Flecainide, propafenone and metoprolol (used in cardiovascular disease), desipramine, clomipramine and nortriptyline (antidepressants) and risperidone, thioridazine and haloperidol (antipsychotics). You may need a dose adjustment Estsitil.
 
Taking Estsitil with food and drink
 
Estsitil can be taken with or without food (see section H "How to take Estsitil").
As with many drugs, Estsitil combined with alcohol is not recommended, although not expected Estsitil to interact with alcohol.
 
Pregnancy and lactation
 
Tell your doctor if you are pregnant or plan to become pregnant. Estsitil Do not take if you are pregnant unless you have discussed the risks and benefits with your doctor.

If your newborn baby has any of these symptoms, please contact their doctor immediately.
 
If used during pregnancy, Estsitil should never be stopped abruptly.
If you are nursing, do not take Estsitil than eye you have discussed the risks and benefits with your doctor.
Ask your doctor or pharmacist before taking any medicine.
 
Driving and operating machinery
 
It is advisable not to drive or operate machinery until you know how you affect Estsitil.
 

 HOW TO TAKE ESTSITIL

 
Always Estsitil exactly as your doctor has told you. If you're not sure, ask your doctor or pharmacist.
 
Adults
 
 Depression
 
Estsitil normally recommended dose is 10 mg, taken once daily. Your doctor may increase the dose to 20 mg per day.
 
 Panic Disorder
 
Estsitil starting dose is 5 mg once daily for a week before the first dose should be increased to 10 mg daily. Your doctor can further increase the dose to 20 mg daily.
  Social Anxiety Disorder
 
Estsitil normally recommended dose is 10 mg, taken once daily. Your doctor may reduce the dose to 5 mg daily or to increase it to 20 mg daily, depending on how you react to this medicine.
  Generalised Anxiety Disorder
 
Estsitil normally recommended dose is 10 mg, taken once daily. Your doctor may increase your dose to 20 mg daily.
  Obsessive Compulsive Disorder
 
Estsitil normally recommended dose is 10 mg, taken once daily. Your doctor may increase your dose to 20 mg daily.
 
Elderly (over 65 years)
 
The usual recommended starting dose Estsitil is 5 mg, taken once daily.
 
Children and adolescents (under 18 years)
 
Usually Estsitil should not be given to children and adolescents.
 
For more information, please refer to 2 "Before you take Estsitil."
 
You can take Estsitil with or without food. Swallow with water. Do not chew, as it has a bitter taste.
If necessary, you can split the tablet by placing it on a flat surface with the score up. Then you can break it by pressing down with the index fingers of both
sides.
 
Duration of treatment
 
It may take several weeks before you feel better. Continue to take Estsitil even take some time before you feel any improvement in your condition. Do not change the dose of your medication without first talking to your doctor.
Continue to take Estsitil as long as your doctor recommends. If you stop treatment too soon, your symptoms may reappear. It is recommended that treatment be continued for at least 6 months after you feel well again.
 
If you take more than the dose Estsitil
 
If you take more than the prescribed dose Estsitil, contact your doctor or nearest emergency department care. Do this even if there are no signs of discomfort. Some signs of an overdose may have dizziness, tremor, agitation, convulsions, coma, nausea, vomiting, change in heart rate, low blood pressure and changes in the balance of fluid and salt balance. Take the box / packaging Estsitil with you when you go to the doctor or hospital.
 
If you forget to take Estsitil
 
Do not take a double dose to make up for missed doses. If you forget to take a dose and you remember before going to bed, take the dose immediately. The next day, proceed as usual. If you only remember during the night or the next day, skip the missed dose and continue as usual.
 
If you stop taking Estsitil
 
Do not stop taking Estsitil until your doctor tells you to do so. When you finish the course of treatment is generally recommended dose Estsitil be reduced gradually over several weeks.
When you stop taking Estsitil, especially if suddenly, you may experience withdrawal symptoms. They are usually seen in discontinuation Estsitil. The risk is greater when Estsitil was administered for long periods or in high doses or when the dose is reduced too quickly. According to most people the symptoms are mild and resolve themselves is within two weeks.
 
In some patients, however, may be strong or prolonged (2-3 months or more). If you show strong withdrawal symptoms when you stop taking Estsitil, please contact your doctor. He may prescribe you start taking the tablets again and come off them more slowly.
 
Withdrawal symptoms include: dizziness (instability or lack of balance) prickling, burning, and (more rarely) a sense of shock, including the head, sleep problems (vivid dreams, nightmares, inability to sleep), anxiety, headache, nausea (nausea), sweating (including night), nervousness and excitability, tremors (shaking), confusion or disorientation, emotional or irritable, diarrhea (loose stools), visual problems, irregular or pounding heartbeat (palpitations).

 POSSIBLE SIDE EFFECTS

 
Like all medicine Estsitil can cause side effects, although not everybody gets them. Side effects usually disappear after a few weeks. You should know that many reactions can also be symptoms of your illness and therefore will improve when you start healing.
 
See a doctor if you experience any of these side effects during treatment:
 
Uncommon (more than 1 in 1000 people and less than 1 in 100 people):
 
 Unusual bleeding, including gastrointestinal (including rectal) bleeding.
 
Rare (more than 1 in 10,000 and less than 1 in 1000 people):
 If you get swelling of the skin, tongue, lips or face, or difficulty breathing or swallowing (allergic reaction), contact your doctor or go to
hospital.
 If you have a fever, agitation, confusion, trembling and abrupt cuts in the muscles, it may be signs of a rare condition called serotonin syndrome. If you feel this way, contact your doctor.
 
If you experience the following side effects should immediately contact your doctor or go to hospital:
 Difficulty in urinating;
 Seizures (fits), see also section "Take special care with Estsitil"
 Yellowing of the skin and whites of the eyes are symptoms of hepatic impairment / hepatitis.
 
In addition to the above, reported the following adverse reactions:
 
Very common (more than 1 in 10 people):
 Feeling sick (nausea).
 
Common (more than 1 in 100 and less than 1 in 10 people):
 Nasal congestion or runny nose (sinusitis);
 Decreased or increased appetite;
 Anxiety, agitation, abnormal dreams, difficulty sleeping, drowsiness, dizziness, yawning, tremors, pricking the skin;
 Diarrhea, constipation, vomiting, dry mouth;
 Increased sweating;
 Pain in muscles and joints (arthralgia and myalgia);
 Sexual dysfunction (delayed ejaculation, erectile dysfunction, reduced sexual desire in women - study hard orgasm);
 Fatigue, fever;
 Increased weight.
 
Uncommon (more than 1 in 1000 people and less than 1 in 100 people):
 Hives (urticaria), rash, itching (pruritus);
 Grinding of teeth, excitability, nervousness, panic attacks, confusion;
 Disturbed sleep, impaired taste, fainting (syncope);• Large pupils (mydriasis), blurred vision, ringing in the ears (tinnitus);
 Loss;
 Vaginal bleeding;
 Reduced weight;
 Increased heart rate;
 Swelling of the hands or feet;
 Bleeding from the nose.
 
Rare (more than 1 in 10,000 and less than 1 in 1000 people):
 Aggressive, depersonalization, hallucinations;
 Slow pulse.
 
Some patients reported (frequency not known):
 Increased level of sodium in the blood (the symptoms are nausea and discomfort with muscle weakness or confusion);
Dizziness when standing due to low blood pressure (orthostatic hypotension);
 abnormal liver function (increased levels of liver enzymes in the blood);
 Movement disorders (involuntary movements of the muscles);
 painful erection (priapism)
 Disorders associated with bleeding, including the skin and mucous membranes (ecchymosis) and low platelet count (thrombocytopenia);
 Sudden swelling of the skin or mucous membranes (angioedema);
 Increase the amount of excreted urine (unusual secretion of antidiuretic hormone);
 leakage of milk from the breasts in women who do not breastfeed;
 Mania.
 
Thoughts of harming or killing themselves (some cases of considering suicide and suicidal behavior during treatment with escitalopram or premature discontinuation (see also section "Take special care with Estsitil").
 
In addition, several known side effects that occur with the use of drugs acting in a similar way to escitalopram (the active ingredient in Estsitil). They are:
Motor restlessness (akathisia);
 Anorexia.
 
For patients taking this type of medication is increased risk of fractures (breaks) in the bones.
 
If any of the side effects gets serious, or you notice other effects not listed in this leaflet, please tell your doctor or pharmacist.
 

 HOW TO STORE ESTSITIL

 
Keep out of reach of children.
No special storage conditions. Estsitil Do not use after the expiry date stated on the carton and blister after "EXP". The expiry date refers to the last day of that month.
 
Medicines should not be disposed of via wastewater or household waste. Ask your pharmacist how to dispose of medicines no longer required. These measures will help to protect the environment.

  Active substance: escitalopram (escitalopram). Each tablet contains Estsitil 5 mg, 10 mg, 15 mg or 20 mg escitalopram (as oksalot).
  Excipients
 
Core: Microcrystalline cellulose (E 460), croscarmellose sodium (E 468), silica colloidal anhydrous, magnesium stearate (E 470Y).
