ESCITIL WHAT IS AND WHAT IT IS USED
Escitil comprises escitalopram (escitalopram) is used for the treatment of depression ( major depressive disorder ) and anxiety disorders (e.g., panic disorder with or without agoraphobia , i.e. fear of places where it can not support , social anxiety disorder , generalized anxiety disorder, and obsessive compulsive disorder ) .
Escitalopram belongs to a class of antidepressants called selective inhibitors of serotonin reuptake (SSRIs). These drugs act on the serotonin system in the brain by increasing the level of serotonin. Disturbances of the serotonin system is considered to be an important factor in the development of depression and related diseases .
2 . BEFORE YOU TAKE ESCITIL
Do not take Escitil
• if you are allergic (hypersensitive) to estsitaloprom or any of the other ingredients of Estsitil (see section 6 "Further information").
• if you are taking other medicines that belong to a group called MAO inhibitors, including selegiline (used to treat Parkinson's disease) , moclobemide (used to treat depression) and linezolid ( an antibiotic).
Take special care with Estsitil
Please tell your doctor if you suffer from another condition or disease as may be necessary for your doctor to take this into account .
In particular, tell your doctor :
• if you suffer from seizures for the first time in your life or have epilepsy . Estsitil therapy should be discontinued if seizures occur or if there is an increase in seizure frequency (see also section 4 " Possible side effects").
• if you suffer from liver or kidney function. You need to your doctor to adjust your dose .
• if you have diabetes . Treatment with Estsitil may alter glycemic control. It may be necessary to adjust the dose of insulin and / or oral hypoglycaemic drugs.
• if you have a reduced level of sodium in the blood .
• if you have a tendency to bleed easily or bruising .
• if you are receiving electroconvulsive treatment.
• if you have coronary heart disease.
Please note
As with other drugs used for treatment of depression or related disorders , the improvement is not achieved immediately . After initiation of therapy with Estsitil may take several weeks before you feel any improvement . In the treatment of panic disorder improvement is usually seen after 2-4 weeks. At the beginning of treatment , some patients may experience increased anxiety which will disappear with longer treatment . It is therefore very important to carefully follow your doctor's instructions and do not stop treatment or change the dose without consulting him.
Thoughts of suicide and worsening of your depression or anxiety disorder.
If you are depressed and / or have anxiety disorders you can sometimes have thoughts of harming or killing themselves. These may be increased when first starting antidepressants , so while these medicines all take time to start running - usually about two weeks but sometimes longer.
You may be more likely to think like this :
• If you have previously had thoughts about killing or harming yourself .
• If you are a young man. Data from clinical studies suggest an increased risk of behavior associated with suicide in young people ( under 25 years) with psychiatric conditions who were treated with an antidepressant.
If at any time until you get any thoughts of harming or killing yourself , contact your doctor or go to hospital.
You may find it helpful to tell a relative or close friend that you are depressed or have an anxiety disorder , and ask them to read this leaflet . You might ask them to tell you if they think your depression or anxiety is getting worse, or are worried about changes in your behavior.
Some patients with manic depressive illness may enter into a manic phase. It is characterized by unusual and rapidly changing ideas , inappropriate happiness and excessive physical activity. If you experience any of these conditions , contact your doctor. Symptoms such as restlessness or difficulty sitting or standing in one place can also occur during the first weeks of treatment. Tell your doctor immediately if you experience these symptoms.
Sometimes you may not be aware of the above symptoms and therefore you can to be helpful to ask a friend or relative to help you keep track of possible signs of change in your behavior .
Tell your doctor immediately or contact your nearest hospital , if you have disturbing thoughts or experiences , or if any of the above symptoms occur during therapy .
Use in children and adolescents under 18 years of age
Usually Escitil should not be used in children and adolescents under 18 years of age.
You should also know that patients under 18 have an increased risk of side effects such as suicide attempt , suicidal thoughts and hostility (mostly aggression, oppositional behavior and anger ) when they take this class of drugs . However, your doctor may prescribe Estsitil patients under 18 years old when he decided that it is in their interest. If your doctor has prescribed Estsitil a patient under 18 and you want to discuss this , please go back to your doctor. You must tell your doctor if any of the above symptoms occur or worsen when patients under 18 receive Estsitil . Also current long-term safety consequences on growth , maturation and cognitive and behavioral development of Estsitil in this age group have not yet been demonstrated.
Taking other medicines
Please tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including medicines obtained without a prescription.
Tell your doctor if you are taking any of these medicines:
• " Non-selective monoamine oxidase inhibitors ( MAOIs ) " containing phenelzine, iproniazid , isocarboxazid , nialamide and tranylcypromine as active substances. If you are taking any of these medicines, you have to wait 14 days before you start taking Estsitil . After stopping Estsitil must pass seven days before taking any of these medicines .
• " Reversible selective MAO-A inhibitors" , containing moclobemide (used in the treatment of depression ) .
• " Irreversible MAO-B inhibitors" , containing selegiline (used in the treatment of Parkinson's disease) . They increase the risk of side effects .
• antibiotic linezolid .
• Lithium (used in the treatment of manic depressive disorder ) and tryptophan.
• Imipramine and desipramine ( both are used for the treatment of depression ) .
• Sumatriptan and similar drugs (used to treat migraine) and tramadol (used against severe pain) . They increase the risk of side effects.
• Cimetidine and omeprazole (used to treat stomach ulcers) , fluvoxamine ( antidepressant ) and ticlopidine (used to reduce the risk of stroke). They can cause elevated blood levels of Estsitil .
• St. John's wort (hypericum perforatum) - a herbal medicinal product used for depression .
• Aspirin and non-steroidal anti-inflammatory drugs (medicines used to relieve pain or to thin the blood , so called anticoagulants) . They can increase the tendency to bleed .
• Warfarin , dipyridamole , and phenprocoumon (medicines used to thin the blood, so called anticoagulants) . Your doctor will probably check the clotting time of your blood at the commencement and termination of Estsitil to check whether your dose anticoagulants is still sufficient.
• Mefloquine (used for the treatment of malaria ) , bupropion (used for the treatment of depression ) and tramadol (used to treat severe pain ) , because of the possible risk of lowering seizure threshold.
• Neuroleptics (medicines to treat schizophrenia , psychosis ) because of the possible risk of lowering the seizure threshold , and antidepressants.
• Flecainide , propafenone and metoprolol (used in cardiovascular disease ) , desipramine, clomipramine and nortriptyline (antidepressants ) and risperidone, thioridazine and haloperidol ( antipsychotics ) . You may need a dose adjustment Estsitil .
Taking Escitil with food and drink
Estsitil can be taken with or without food ( see section D " How to take Estsitil " ) .
As with many medicines, combining Estsitil alcohol is not recommended , although it is not expected Estsitil interact with alcohol.
Pregnancy and lactation
Tell your doctor if you are pregnant or plan to become pregnant. Estsitil not take if you are pregnant unless you have discussed the risks and benefits with your doctor.
Make sure your midwife / doctor know that you are taking Estsitil . Given during pregnancy , especially in the last months of her H , drugs such Escitil can increase the risk
from occurring but serious condition in the newborn , called persistent pulmonary hypertension in the newborn , which causes shortness of breath and blueness of the baby. These symptoms usually occur within the first 24 hours after birth. If this happens to your baby , contact your midwife and / or doctor.
If you take Estsitil in the last 3 months of your pregnancy , you should know that the newborn may occur to the following consequences : difficulty breathing, bluish skin , seizures , changes in body temperature, feeding difficulty , vomiting, low blood sugar, stiff or floppy muscles , vivid reflexes, tremor, jitteriness , irritability , lethargy, constant crying , somnolence and difficulty sleeping .
If your newborn baby has any of these symptoms , please contact their doctor immediately .
If used during pregnancy Estsitil should never be terminated abruptly .
If you are breastfeeding , do not take Estsitil than eye you have discussed the risks and benefits with your doctor .
Ask your doctor or pharmacist before taking any medicine.
Driving and using machines
It is advisable not to drive or operate machinery until you know how you affect Estsitil .
3 . HOW TO TAKE ESCITIL
Always take Estsitil exactly as your doctor tells you . If you are not sure, ask your doctor or pharmacist.
adults
• Depression
The usual recommended dose is Estsitil 10 mg, taken once daily. Your doctor may increase the dose to a maximum of 20 mg per day .
• Panic disorder
Estsitil starting dose is 5 mg once daily for the first week before the dose be increased to 10 mg daily. Your doctor may further increase the dose to a maximum of 20 mg daily.
• Social Anxiety Disorder
The usual recommended dose is Estsitil 10 mg, taken once daily. Your doctor may reduce your dose to 5 mg daily, or increase it to a maximum of 20 mg per day depending on how you react to this medicine.
• Generalised Anxiety Disorder
The usual recommended dose is Estsitil 10 mg, taken once daily. Your doctor may increase your dose to a maximum of 20 mg daily.
• Obsessive Compulsive Disorder
The usual recommended dose is Estsitil 10 mg, taken once daily. Your doctor may increase your dose to a maximum of 20 mg daily.
Elderly (over 65 years)
The usual recommended starting dose Estsitil 5 mg, taken once daily.
Children and adolescents (under 18 years)
Usually Estsitil should not be given to children and adolescents.
For more information, please see Item 2 " Before you take Estsitil ."
You can take Estsitil with or without food. Swallow with water. Do not chew , as a bitter taste.
If necessary , you can split the tablet by placing it on a flat surface with the score up . Then you can break it by pressing down with your index both
sides.
Duration of treatment
It may take several weeks before you feel better. Continue to take Estsitil even take some time before you feel any improvement in your condition . Do not change the dose of your medication without first talking to your doctor.
Continue to take Estsitil as long as your doctor recommends . If you stop treatment too soon , your symptoms may reappear. It is recommended to continue treatment for at least 6 months after you feel well again .
If you take more dose Estsitil
If you take more than the prescribed dose Estsitil , contact your doctor or nearest casualty department assistance. Do this even if there are no signs of discomfort. Some of the signs of overdose may have dizziness, tremors, excitability, convulsions , coma, nausea , vomiting, change in heart rate, low blood pressure and change in the balance of body fluids and electrolyte balance. Take the box / packaging Estsitil with you when you go to the doctor or hospital.
If you forget to take Estsitil
Do not take a double dose to make up for missed doses . If you forget to take a dose and you remember before going to bed , take the dose immediately . The next day, proceed as usual. If you only remember during the night or the next day, skip the missed dose and continue as usual.
If you stop taking Estsitil
Do not stop taking Estsitil until your doctor tells you to do so. When you finish the course of treatment is generally recommended dose Estsitil be gradually reduced over a period of several weeks.
When you stop taking Estsitil , especially if suddenly , you may experience withdrawal symptoms. They are usually encountered in the discontinuation of Estsitil . The risk is greater when administered Estsitil long time or at high doses, or when the dose is reduced too quickly . According to most people the symptoms are mild and resolve on their own within a couple of weeks.
In some patients , however, may be strong or prolonged (2-3 months or more). If you experience strong withdrawal symptoms when you stop taking Estsitil , please contact your doctor. He may prescribe you start taking the tablets again and come off them slowly.
Withdrawal symptoms include: dizziness ( instability or imbalance ) , stabbing, burning and ( less often) a feeling of electric shock , including in the head, sleep problems (vivid dreams, nightmares , inability to sleep) , anxiety, headache , nausea (nausea) , sweating (including night) , nervousness and excitability, tremors (shaking ), confusion or disorientation, emotional or irritable, diarrhea ( loose stools ) , visual problems , irregular or pounding heartbeat ( palpitations) .
If you have any further questions on the use of this product, ask your doctor or pharmacist.
4 . POSSIBLE SIDE EFFECTS
Like all medicines, Escitil can cause side effects, although not everybody gets them . Side effects usually disappear after a few weeks of treatment. You should know that many of the reactions may also symptoms of your illness and therefore will improve when you start healing.
See a doctor if you experience any of these side effects during treatment :
Uncommon (more than 1 in 1000 people and less than 1 in 100 people) :
• Unusual bleeding, including gastrointestinal (including rectal ) bleeding.
Rare (more than 1 in 10,000 and less than 1 in 1,000 people):
• If you experience swelling of the skin, tongue , lips or face , or difficulty breathing or swallowing ( allergic reaction) , contact your doctor or go
hospital.
• If you have a fever , excitability , confusion , trembling and abrupt cuts in the muscles , it may be signs of a rare condition called serotonin syndrome . If you feel this way , contact your doctor.
If you get these side effects you should immediately contact your doctor or go to hospital :
• Difficulty urinating ;
• Seizures ( fits) , see also section " Take special care with Estsitil "
• Yellowing of the skin and whites of the eyes are signs of hepatic impairment / hepatitis.
In addition to the above, reported the following side effects:
Very common (more than 1 in 10 people):
• Feeling sick ( nausea) .
Common ( more than 1 in 100 people and less than 1 in 10 people ):
• stuffy or runny nose ( sinusitis ) ;
• increased or decreased appetite;
• Anxiety , restlessness , abnormal dreams , difficulty sleeping, drowsiness , dizziness , yawning, tremors, prickling of the skin;
• diarrhea , constipation , vomiting , dry mouth ;
• increased sweating ;
• Pain in muscles and joints (arthralgia and myalgia) ;
• Sexual dysfunction ( delayed ejaculation, erectile dysfunction , reduced sexual desire in women - study hard orgasm ) ;
• Fatigue , fever ;
• Increased weight.
Uncommon (more than 1 in 1000 people and less than 1 in 100 people) :
• Hives (urticaria ), rash, itching ( pruritus) ;
• Grinding of teeth , excitability, nervousness, panic attacks , confusion ;
• Disturbed sleep , abnormal taste , fainting (syncope ) ;
• Dilated pupils ( mydriasis ) , impaired vision , ringing in the ears ( tinnitus) ;
• Loss ;
• Vaginal bleeding ;
• Reduced weight ;
• Increased heart rate ;
• Swelling of the hands or feet ;
• Bleeding from the nose.
Rare (more than 1 in 10,000 and less than 1 in 1,000 people):
• aggression, depersonalization , hallucinations ;
• Slow pulse .
Some patients reported (frequency not known):
• Increased level of sodium in the blood ( the symptoms are nausea and discomfort with muscle weakness or confusion) ;
• dizziness when standing due to low blood pressure (orthostatic hypotension) ;
• abnormal liver function tests (increased levels of liver enzymes in the blood) ;
• Movement disorders ( involuntary movements of the muscles) ;
• painful erection ( priapism )
• disorders , bleeding , including the skin and mucous membranes ( ecchymosis ) and low platelet count (thrombocytopenia ) ;
• Sudden swelling of the skin or mucous membranes ( angioedema ) ;
• Increase the amount of excreted urine (unusual secretion of antidiuretic hormone) ;
• Leakage of breast milk in women who did not breastfeed ;
• Manila.
Thoughts of harming or killing themselves (reported cases of considering suicide and suicidal behavior during treatment with escitalopram or premature discontinuation (see also section " Take special care with Estsitil " ) .
Furthermore, are known and several side effects that occur with the use of drugs acting by similar way of escitalopram ( the active ingredient of Estsitil ) . They are:
• Motor restlessness ( akathisia ) ;
• Anorexia .
In patients taking this type of medicine is an increased risk of fractures ( breaks) of the bones.
If any of the side effects gets serious , or you notice other effects not listed in this leaflet , please tell your doctor or pharmacist.
5 . HOW TO STORE ESCITIL
Keep out of reach of children.
No special storage conditions. Estsitil not use after the expiry date stated on the carton and blister after " EXP " . The expiry date refers to the last day of that month.
Medicines should not be disposed of via wastewater or household waste. Ask your pharmacist how to dispose of medicines no longer required . These measures will help to protect the environment.
6 . ADDITIONAL INFORMATION
What does Escitil :
• Active substance : Escitalopram (escitalopram). Each tablet Estsitil comprises 5 mg, 10 mg, 15 mg or 20 mg escitalopram (as oksalot ) .
• Excipients
Core: Cellulose, Microcrystalline (E 460 ), croscarmellose sodium ( E 468 ) , silica , colloidal anhydrous , magnesium stearate ( F 470Y ) .
Coating: Hypromellose ( E464 ), titanium dioxide (E 171 ) and macrogol 400 .
Estsitil looks like and contents of pack
Estsitil preposition is in the form of 5 mg, 10 mg, 15 mg and 20 mg tablets . The tablets are described below :
Estsitil 5 mg: Round , white , film-coated tablet.
Estsitil 10 mg: oval (about 8,1 x 5,6 mm), white , film-coated tablet , scored on one side. The tablet can be divided into equal halves.
Estsitil 15 mg: oval (about 10,4 x 5,6 mm), white , film-coated tablet , scored on one side. The score line is used only to facilitate breaking for ease of swallowing and not to divide into equal doses.
Estsitil 20 mg: oval (about 11,6 x 7,1 mm), white , film-coated tablet , scored on one side.
The tablet can be divided into equal halves.
Estsitil is available in the following sizes: 7 , 10, 14 , 15, 20, 28 , 30, 30 x 1 , 49, 49 x 1 , 50, 56, 60 , 84, 90, 98 , 100 , 100 x 1 200 500 film-coated tablets ( blisters).
